허약하거나 고령인 대상자를 돌본 1년 이상의 경력 필수
재가, 지역사회 또는 장기요양 환경에서 2년 이상의 전문 간호(Skilled Nursing) 경력 우대
재가, 지역사회 또는 장기요양 환경에서 Home Health Aide(HHA), CNA 또는 돌봄 인력을 직·간접적으로 관리한 경험 우대
주요 업무 및 책임
홈케어 관리 및 서비스 조정
홈케어 부서 및 서비스의 일상적인 운영을 효과적으로 관리하고 리더십 발휘
참가자의 가정을 방문하여 초기 평가를 실시하고, 개인별 필요 사항, 현재 이용 가능한 지원 체계, 주거 환경의 안전성 및 가정 내 추가 지원 서비스 필요 여부를 적시에 평가
정기 및 필요 시 재평가를 실시하여 홈케어 서비스의 필요성과 계획된 서비스 제공 현황을 확인
IDT(다학제 팀) 및 가정 내 돌봄 지원 인력(가족, 친구 등)과 협력하여 홈케어 케어 플랜(Plan of Care) 수립
정기 케어 플랜 회의에 참여하여 참가자의 케어 플랜 수립에 기여
IDT 회의에 정기적으로 참석하여 참가자의 상태 변화를 공유하고, 케어 플랜 관련 의사결정에 협력하며, 24시간 케어 제공을 조율
돌봄 인력 및 홈케어 스케줄러를 대상으로 오리엔테이션과 초기·연례 역량 평가를 실시하고, 업무를 감독하며 성과를 평가
홈케어 직원이 케어 플랜에 따라 참가자에게 전문적인 지원 및 회복 간호 서비스를 제공하도록 관리
홈케어 직원이 참가자의 의무기록을 정확하게 관리하고 필요한 차팅 및 보고 요건을 준수하도록 관리
홈헬스 간호 직원이 필요에 따라 참가자와 가족에게 개별 또는 그룹 교육 및 훈련을 제공하도록 관리
모든 참가자에게 제공되는 가정 내 홈케어 서비스 전반을 조율
K-day PACE와 계약된 Home Health Agency 간의 연락 및 조정 담당
참가자의 홈케어 필요에 맞춰 적절한 인력이 배치될 수 있도록 인력 수요와 활용을 관리하고 우선순위를 정하여 관련 정보를 IDT에 공유
계약된 홈헬스 기관의 모든 직원이 초기 및 연례 역량 평가를 완료하고 CMS 및 K-day PACE의 교육·인사 관련 요건을 충족하도록 관리
승인된 홈케어 서비스의 이용 데이터를 관리하고, 홈케어 직원 및 독립 계약자를 위한 신규 직원 오리엔테이션, 교육 및 지속적인 직무교육 프로그램의 개발과 운영에 참여
필요한 경우 참가자의 가정에서 직접 전문 간호(Skilled Nursing) 서비스 제공
온콜(On-call) 업무에 참여
모든 기관 정책과 절차 및 OSHA 안전 지침 준수
HIPAA 규정에 따라 참가자, 고객 및 가족에 관한 정보와 회사의 모든 절차 및 정보를 철저히 기밀로 유지
감염 가능성이 있는 검체 취급 시 적절한 생물학적 위험물질(Biohazard) 예방 조치를 취하고 표준주의지침(Standard Precautions)을 준수
품질 개선 활동에 참여하고 지원
지속적인 교육 과정과 필수 직원 회의 및 교육에 참여하고, 전문기관과의 관계 및 필요한 자격증을 유지
본인의 면허 및 권한에 허용된 업무 범위 내에서만 업무 수행
기타 필요하거나 요청되는 업무 수행
급여 및 복리후생

Qualifications and Requirements:
Education/Training/Certifications:
Active LVN license in good standing
Current CPR, AED, and First Aid Certification required
Current driver’s license and proof of auto insurance
Experience:
Must have a year of experience with a frail or elderly population
Minimum of 2 years of skilled nursing experience in a home-based, community, or long-term care setting preferred
Prior experience managing, directly or indirectly, Home Health Aides, CNAs, or caregivers in a home-based, community, or long-term care setting preferred
Duties and Responsibilities:
Receptionist / Scheduler / Medical Records – Key Duties
Demonstrate effective leadership in managing daily operations of the home care department and services.
Perform initial assessments of participants in their home environment to determine individual needs, assess current support systems, safety of the home environment, and need support services in the home in a timely manner.
Perform scheduled and unscheduled re-assessments to assess home care service needs and provision of services as planned.
Develop a home care plan of care in coordination with the Interdisciplinary Team and caregiving resources in the home (family, friends, etc.).
Participate in the development of the participant’s plan of care at scheduled plan of care meetings.
Maintain regular attendance at, and participate in, interdisciplinary team meetings, communicating participant changes, collaborating care planning decisions, and coordination of 24-hour care delivery.
Provide orientation, initial and annual competencies, supervise, and evaluate performance of caregivers and home care schedulers.
Assure that home care staff provide skilled supportive and restorative nursing care to participants as prescribed in the home care plan of care.
Assure that home care staff maintain participants’ medical records and fulfill charting and reporting requirements.
Assure that home health nursing staff provide education and training to participants and their families on an individual or group basis as needed.
Responsible for the coordination of home care services for all participants in the home.
Serve as a liaison between K-day PACE and contracted home health agencies.
Manage and prioritize utilization of homecare staffing needs to reflect enrollee homecare needs. Provide this information to the interdisciplinary team.
Ensure all contracted home health agency staff complete initial and annual competency testing and fulfill all CMS and K-day PACE training and personnel requirements.
Maintain utilization data for authorized home care services provided. Participate in developing and organizing new staff orientation, training and on-going in-service training for home care staff and independent contractors.
Provide skilled nursing care in the home when needed.
Participate in the on-call process.
Follow all policies and procedures and OSHA safety guidelines.
Maintain the confidentiality of all company procedures and information about participants, clients or families according to HIPAA regulations.
Handle potentially infectious specimens with appropriate biohazard precautions and practice standard precautions.
Participate in and support Quality Improvement initiatives.
Participate in continuing education classes and any required staff and training meetings. Maintain professional affiliations and any required certifications.
Act only within the scope of his or her authority to practice.
Perform other duties as required or requested.
