K-day PACE는 한인타운을 중심으로 한인 어르신들에게 종합적인 의료 및 생활 지원 서비스를 제공하는 PACE 프로그램입니다. 교통편, 식사, 다양한 프로그램과 활동, 진료, 치과 진료, 물리치료 등 어르신들에게 필요한 서비스를 한 곳에서 통합적으로 제공하고 있습니다.

자격 요건
학력 / 교육 / 자격증
고등학교 졸업 또는 이에 준하는 학력 필수
유효한 CPR, AED 및 응급처치(First Aid) 자격증 필수
유효한 운전면허증 및 자동차 보험 가입 증명서 필수
참가자를 독립적으로 돌보기 전에 K-day PACE 오리엔테이션, caregiver 교육 및 해당 직무에 필요한 역량 평가를 성공적으로 이수해야 함
신원조회, 고용 자격 확인, 건강검진 등 관련 채용 요건을 모두 충족해야 함
경력
고령자 또는 도움이 필요한 어르신을 돌본 1년 이상의 경력 필수
케어기빙, 개인 돌봄, IHSS, 홈케어, 시니어 케어, CNA/HHA, 지역사회 기반 케어 또는 장기요양 관련 경력자 우대
케어기빙 또는 재가·지역사회 기반 노인 돌봄 분야 2년 이상 경력자 우대
주요 업무 및 책임
참가자 돌봄 및 일상생활 지원
목욕, 개인위생, 옷 입기, 화장실 이용, 식사, 이동, 자세 변경, 보행 및 이동 보조 등 일상생활활동에 필요한 직접적인 개인 돌봄 및 지원 제공
승인된 범위 내에서 참가자 관련 세탁, 간단한 집안일, 필요 시 장보기 및 간단한 식사 준비 등 도구적 일상생활활동 지원
참가자의 케어 플랜에 따라 말벗, 관찰 및 감독, 일상 활동을 위한 안내와 필요한 지원 제공
참가자의 존엄성, 사생활, 안전 및 개인의 선택을 존중하면서 가능한 한 높은 수준의 독립성을 유지할 수 있도록 지원
승인된 케어 플랜과 배정된 업무 목록에 따라 정해진 일정, 서비스 횟수 및 시간에 맞춰 서비스 제공
승인 없이 참가자의 홈케어 서비스나 제공 시간을 임의로 추가·중단하거나 늘리거나 줄이는 등 중대한 변경을 하지 않음
필요한 교육을 이수한 후 K-day 의료진의 지시에 따라 혈압, 체온, 맥박 등 기본 건강 상태를 측정하고 기록
케어 플랜에 포함된 경우, 필요한 교육과 업무 숙련도 확인 후 재활팀의 지침에 따라 참가자별 가정 운동, 이동 및 기능 향상 프로그램 지원
참가자의 건강 상태, 신체 기능, 인지 능력, 이동 능력, 행동, 식욕, 피부 상태, 안전 또는 평소 일상 활동 수행 능력의 변화를 관찰하고, 이상이나 우려 사항이 있을 경우 K-day의 보고 절차에 따라 신속히 보고
낙상, 부상, 참가자의 서비스 거부, 위험한 환경, 중대한 행동 변화 또는 기타 우려 사항 발생 시 해당 K-day 담당자에게 즉시 보고
K-day의 비상 대응 절차를 준수하고 필요한 경우 응급 서비스에 연락
각 방문의 서비스 시작·종료 시간, 제공한 서비스, 완료한 업무, 거부되거나 완료하지 못한 업무 및 필요한 참가자/보호자 확인 사항을 정확하게 기록
결근, 지각, 참가자의 일정 취소, 참가자와 연락이 되지 않는 경우, 일정 충돌 또는 예정된 방문을 완료할 수 없는 상황 발생 시 Home Care Scheduler에게 즉시 보고
K-day Home Care의 승인 없이 돌봄 담당자 간 담당 참가자를 임의로 변경하거나 예정된 방문 일정을 변경하지 않음
참가자의 개인정보와 기밀을 보호하고 HIPAA, 참가자 권리 관련 규정, 전문적인 관계 및 행동 기준, K-day 정책을 준수
학대, 방임, 착취, 유기, 절도 또는 기타 참가자의 안전과 관련된 문제가 의심될 경우 K-day 정책 및 관련 신고 규정에 따라 신속하게 보고
기관 정책, OSHA 요건, 감염관리 기준, 비상대비 절차 및 안전한 업무수칙 준수
필수 교육, 지속적인 직무교육, 업무 숙련도 평가, 품질 개선 활동 및 참가자별 맞춤 교육에 참여
PACE 모델을 적극적으로 지지하고, 재가 및 지역사회 기반 환경에서 어르신 돌봄의 모범 사례를 실천할 수 있도록 지원
배정된 업무에 따라 참가자의 가정 및 K-day 사업장 간 이동
급여 및 복리후생
시급 $23 + 직원 복리후생 제공

Qualifications and Requirements:
Education/Training/Certifications:
High school diploma or equivalent required.
Current CPR, AED, and First Aid Certification required
Current driver’s license and proof of auto insurance
Must successfully complete K-day PACE orientation, caregiver training, and position-specific competency validation before independently providing participant care.
Must meet all applicable background, exclusion-screening, health-clearance, and other employment requirements.
Experience:
Must have a year of experience with a frail or elderly population
Prior caregiving, personal care, IHSS, home care, senior care, CNA/HHA, community-based care, or long-term care experience preferred.
Two years of caregiving or home/community-based elder-care experience preferred.
Duties and Responsibilities:
Provide hands-on personal care and assistance with Activities of Daily Living (ADLs), including but not limited to bathing, grooming, dressing, toileting, eating, mobility, positioning, ambulation, and transfers.
Assist participants with authorized Instrumental Activities of Daily Living (IADLs), including but not limited to participant-related laundry, light housekeeping, shopping when assigned, and light meal preparation.
Provide companionship, supervision, cueing, and supportive assistance as identified in the participant’s care plan.
Encourage participants to maintain the highest practical level of independence while protecting dignity, privacy, safety, and personal choice.
Follow the participant’s authorized care plan, assigned task list, schedule, frequency, and duration of services.
Do not independently add, discontinue, increase, decrease, or substantially change a participant’s authorized Home Care services or hours.
Measure and document basic vital signs only when specifically instructed by K-day clinical staff and after appropriate training and competency validation.
Assist the Rehabilitation team with participant-specific home exercise, mobility, or functional programs when included in the care plan and after appropriate instruction and competency validation.
Observe participants for changes in condition, function, cognition, mobility, behavior, appetite, skin condition, safety, or ability to perform usual activities and promptly report concerns according to K-day escalation procedures.
Promptly report falls, injuries, participant refusals, unsafe conditions, significant behavioral changes, or other concerns to the appropriate K-day team member.
Follow K-day emergency procedures and contact emergency services when indicated.
Accurately document each visit, including required clock-in/out, services provided, assigned tasks completed, tasks refused or not completed, and required participant/representative verification.
Immediately notify the Home Care Scheduler regarding call-outs, lateness, participant cancellations, inability to reach a participant, schedule conflicts, or inability to complete a scheduled visit.
Do not independently exchange caregiver assignments or modify scheduled visits without approval from K-day Home Care.
Maintain participant confidentiality and comply with HIPAA, participant-rights requirements, professional-boundary expectations, and K-day policies.
Promptly report suspected abuse, neglect, exploitation, abandonment, theft, or other participant-safety concerns according to K-day policy and applicable reporting requirements.
Follow organizational policies, OSHA requirements, infection-control standards, emergency-preparedness procedures, and safe-work practices.
Participate in required training, continuing education, competency evaluations, quality-improvement activities, and participant-specific training.
Support and advocate for the PACE model and best practices in care for older adults in home- and community-based settings.
Travel between participant homes and K-day locations as required by assigned duties.
