Qualifications and Requirements:
Education/Training/Certifications: 
High school diploma or equivalent required
Current CPR, AED, and First Aid Certification required
Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred
Experience:
Prior experience in healthcare and medical records is preferred.

Duties and Responsibilities:
Receptionist / Scheduler / Medical Records – Key Duties
Manage front desk operations: answer calls promptly, greet visitors, route inquiries, handle mail, maintain directories, and support check-in/check-out.
Provide professional customer service to participants, families, staff, and providers; respond to emergencies per procedures.
Perform general clerical tasks (filing, faxing, copying) and maintain organized communication and support lists.
Coordinate scheduling for clinic visits, assessments, and ongoing care; send reminders, manage changes, and resolve scheduling issues in collaboration with the IDT and participants.
Prepare charts, support Plan of Care and reassessment scheduling, and arrange future appointments as ordered.
Maintain medical records: create and manage charts, scan and organize documents, request/send records, and ensure accurate demographics.
Protect confidentiality and comply with HIPAA, universal precautions, and participant rights requirements.
Compile reports, meeting documentation, and correspondence; support Quality activities including grievances, appeals, and audit data entry.
Follow organizational policies, OSHA and infection control standards; participate in training, quality improvement, and continuing education.
Support and advocate for the PACE model and best practices in care for older adults in home- and community-based settings.
Salary & Benefits:
$23 - $28/hr + Employee Benefits
