Required:
- Holds a master's degree in psychology, gerontology, or counseling from a accredited school
OR
- Holds a bachelor's degree in social work, or social science major from an accredited school with two years of experience providing social services in one or more of the fields of aging, health, or long-term care services.
OR

-No license required, no experience necessary
-Must have working visa or US citizenship to legally work in US
-Should reside close to Koreatown, Los Angeles for commute
-Bilingual Korean/English speaking preferred

-7am-3:30pm Must be Monday-Friday (can be provided flexible hours)
- Physical clearance from doctor for reaching, bending, stooping crouching, kneeling and grasping; have full range of motion; ability to lift and transfer; must have current First Aid and CPR clearance; proof of CA driver’s license; proof of citizenship

Responsibilities:
1. Interview and screen all participants (ptps) to create individually planned assessments
2. Provide referrals for participants not appropriate for adult day health care.
3. Provide signed and dated documentation for all service performed the day the service is rendered and include signed and dated quarterly progress reports in each ptp’s record.
4. Provide for periodic reevaluation and revision of the individual plan of care.
5. Provide counseling and referral to available community resources.
6. Promote peer group relationships through problem-centered discussion group and task oriented committees.
7. Serve as liaison with other community agencies who may be providing services to a ptp and work with these agencies to coordinate all services delivered to the ptp to meet the ptp’s needs and avoid duplication.
8. Provide discharge planning for all discharged ptps.
9. Act as a member of the assessment team to evaluate the needs of all ptps and develop a plan of care, including short-term and long-term goals, with the participation of ptp’s family and other appropriate individuals.
10. All other services traditionally performed by a social worker and any general business of the center that comes up for which the social worker is adept.
