**자격 요건**
학력 :
-고등학교 졸업 또는 GED 필수
-건강 관련 분야의 준학사 또는 학사 학위 우대
-노인학 교육 또는 훈련 우대
경력 :
-의료 환경에서 마케팅/홍보, intake 또는 영업 역할에서 최소 2년 이상의 경력
-학위가 없는 경우 최소 4년 이상의 경력
주요 업무 및 책임 :
-PACE 프로그램에 대한 정보를 받아들여 제공하고, 필요에 따라 다른 적합한 자원에 연결
-적절한 추적 방법 및 전자 건강 기록(EHR)에 관련 정보를 입력하여 적시 처리가 되도록 함
-잠재적 참여자의 의사와 제공된 추천 서비스 포함하여, 현재의 서면 평가 및 intake 기록 유지
-PACE 프로그램의 자격 요건에 대해 참여자 및 보호자에게 정보 제공
-평가 및 잠재적 참여자 등록 상태와 관련된 일정 및 기한을 마케팅 및 등록 부서, PACE 프로그램 리더십, IDT(Interdisciplinary Team)와 소통
급여 및 복지:
-시급 $25 - $32

Enrollment Specialist Position
Exciting news! A full-service Medicare program, PACE, is finally opening in Koreatown — bringing our Korean seniors everything they need in one place: rides, meals, activities, doctor visits, dental care, physical therapy, and more.

**Qualifications and Requirements**
Education :
High School Diploma or GED required.
Associates or Bachelor’s degree in health-related fields preferred.
Education or training in geriatrics is preferred.
Experience :
-Minimum 2 years of experience in an outreach/marketing, intake or sales role in a health care setting.
-Minimum of 4 years’ experience if no degree.
Duties and Responsibilities :
-Receives and provides information to referrals to the PACE Program; refers individuals to other appropriate resources as indicated.
-Accepts referrals and inputs appropriate information into appropriate tracking methodology(s) and EHR timely.
-Maintains current, written assessment and intake records inclusive of potential participant’s expressed wishes and the referral services provided, as appropriate.
-Communicates timelines and deadlines for assessments and prospective participant enrollment status within the marketing and enrollment department, with PACE program leadership and the IDT.
-Participates in Interdisciplinary Team meetings when potential enrollees are assessed for enrollment into the program for purposes of providing and clarifying information obtained during the enrollment process.
